When you are a patient at Pinsky Family and Sports Medicine Center you will be seen by Mark Pinsky MD. We do not overbook so that we can devote the necessary amount of time to completely evaluate and treat each patient. Therefore please give us as much notice as possible if you are unable to make your scheduled appointment. This will allow us to schedule someone off of our waiting list. While we realize that unexpected events do occur, if you no-show or cancel your visit less than 24 hours in advance, you may be responsible for a cancellation fee. How to Prepare for Your First Appointment

When you come to your first appointment with Pinsky Family and Sports Medicine Center, you are encouraged to download and bring the completed Patient Registration, Patient Information and Medical Release forms to your visit. Please also review the Disclosure of Health Information Policy and the Financial Policy forms. Finally please download and bring the completed Authorization Signature form.

Alternatively, if you are unable to download our forms or would just rather complete a paper version, please note that we do have new patient packets available at our front desk. You are welcome to arrive at least 30 minutes prior to your scheduled appointment time so that you can complete these forms in our office.

  • Arrive on time for your appointment.

  • Be prepared to fill out patient intake paperwork

  • Schedule with office and staff to be prepared.
